Jessica Hurtado, LMSW is a Care Manager in the Health Home Care Management program at Blythedale Children’s Hospital. Jessica earned her Bachelor of Social Work from Mercy College in Dobbs Ferry, graduating Summa Cum Laude, and her Master of Social Work from New York University.
In her current role as Care Manager in the Health Home Care Management program, Jessica coordinates care for children with complex medical needs. She works closely with the children, caregivers, and providers to help families access services, navigate multiple complex systems, and find support.
