Crystal Jones, LMSW is a Care Manager in the Health Home Care Management program at Blythedale Children’s Hospital. Crystal earned her Bachelor of Arts from Lehman College with a major in Social Work, and her Master of Social Work with a clinical focus from the Graduate School of Social Work at Fordham University.
Prior to Blythedale, Crystal was a Social Work Care Manager for the Health Home Serving Children program at Mount Sinai Morningside serving children and adolescents with Serious Emotional Disturbances in Manhattan. During her time at Mount Sinai, she provided home and community-based psychoeducation, clinical support, and linkage to care for members and their families. She also provided comprehensive care planning and advocacy for these members to ensure that their needs were met in all milieus. Crystal also has experience working with the homeless population and HIV positive adults in the Bronx.
In her current role as Care Manager in the Health Home program, Crystal coordinates care for children with complex medical needs. She works closely with the children, caregivers, and providers to help families access services, navigate multiple complex systems, and find support.
