Referral Information
Referrals should include a medical summary with medication list, a psychosocial assessment, demographic and insurance information, and a copy of the child’s immunization record if possible.
We encourage, but do not require, a pre-admission tour by the child’s primary caregiver. Free transportation is provided for the tour if needed.
